FIRST® LEGO® League KOREA Participation Consent Form
This consent form is part of the official registration process for participation in the FIRST® LEGO® League KOREA (FLL Korea).
Participants are required to provide accurate information about the team, coach, and participants.
All submitted information will be used solely for the purposes of event operation, team management, certificate issuance, promotion, and record keeping.
In addition, participants under the age of 14 must obtain consent from a legal guardian.
Coaches are required to upload certificates of completion for the following mandatory trainings:
Child Abuse Mandatory Reporting Education
Sexual Violence Prevention Education
Alternatively, a Teacher Legal Mandatory Education Certificate may be submitted in place of the above.
